The SCCS office in suite 340 in the Student Experience Center is open to the public. Our reception area is open and available for in-person drop-ins. We can also be reached by calling (541) 737-3656 Monday-Friday between 9 and 5 p.m. or by email:

SCCS will continue to host an administrative conference meeting with students via Zoom, telephone, or in person.

The university expects that you treat your remote meeting like an in-person meeting. It is expected that you be in a private setting. We will not host meetings while you are driving or not in a confidential environment. The university does not grant you permission to record this meeting.

The code allows for you to bring an advisor of your choice to the conference. The meeting request letter provides you with information on how to include an advisor within the remote conference format.

You can schedule that meeting using our calendar. You will be participating in this meeting via Zoom technology or by telephone. Questions about the Academic Integrity Course or scheduling your follow-up appointment can be directed to Kris Katkus at [email protected].

If the sanctions you have been assigned can be completed independently and remotely (i.e., reflection assignments, journals, Academic Integrity Course), our office expects that you complete those sanctions in the timeframe previously determined and submit them as instructed in your decision letter. If you do not believe that you will be able to complete the sanction in the given time frame, please email our office at [email protected] and we will work with you as needed.

If the sanctions you have been assigned are to be completed with a campus partner (e.g. Student Health Services IMPACT programming, Student Experiences & Engagement, Career Development Center), contact our office at [email protected] and we will work with you to develop a plan to complete your sanctions.

Both the Preferences, Possibilities, and Decisions Workshop and the Socializing Workshops will be hosted using Zoom technology. You will receive an email reminder 48 hours prior to the scheduled workshop. The email will include your Zoom meeting link and the materials you will use throughout the workshop. If you have any questions regarding this format, please email [email protected]

If you need to submit any completed sanction activities, you may submit them via this form.

Please complete the Hold Removal Request Form for assistance with your registration hold.

SCCS staff works closely with many other departments and OSU program staff to ensure that all students can live, work and learn in a mutually satisfying and secure environment.  The Office of Student Conduct makes available online forms for reporting incidents involving students.

The forms below are for non-emergency incidents only. If this is an emergency, please call 911 or you can reach the OSU Department of Public Safety by calling (541) 737-3010.

The SCCS office will continue to process these requests. You can email your requests to [email protected] in either a Word or PDF format, and we will complete these requests within five business days. Please include your name, ID number (birthdate if you do not have the ID number), and where the completed conduct check should be sent. Records checks that are not submitted by a student must be accompanied by a FERPA appropriate release form. Because we are working remotely, we will be sending all completed disciplinary checks by email. Please include the appropriate email address for the recipient of the form.
